Terms of service
Conditions of service at the Dental Suite Karlín dental practice.
Effective from 1 August 2025
1Introduction
These general terms govern the provision of services by Dental Suite Karlín (Dejvice dent s.r.o.), registered office Lyčkovo náměstí 472/8, Prague 8 — Karlín, Company ID 04125126 (the “Provider”). They apply to all patients using the Provider’s services, in particular those booking via the online reservation system.
2Booking and deposit
Patients may book in person, by phone, or via the online reservation system. For online dental hygiene bookings, a deposit of 900 CZK is required. Payment is made exclusively through the payment gateway during the online booking, and the reservation becomes binding only once payment is successful.
3Rescheduling or cancellation
Patients may reschedule or cancel an appointment without forfeiting the deposit if they do so at least 24 hours before the scheduled procedure. The deposit is then transferred to a new appointment or refunded on request. Cancellations within 24 hours of the appointment, or no-shows, result in forfeiture of the deposit as compensation for the reserved time. Cancellations are made by phone at 774 541 475 or by e-mail to dentalsuitekarlin@gmail.com.
4Price and payment
Service prices are set out in the current price list, available on the website and at the practice. The remaining balance after deduction of the deposit is payable after treatment in cash or by card.
5Patient rights and obligations
Patients are required to provide accurate and complete information, to arrive on time, and to inform the Provider of any obstacles without delay.
6Provider rights and obligations
The Provider is required to deliver services in accordance with professional standards and may decline to treat a patient who repeatedly fails to comply with these terms.
7Personal data
Personal data is processed in accordance with the GDPR and Czech law 110/2019 Sb. Details are set out in our Privacy policy.
8Final provisions
These terms are effective from 1 August 2025. The Provider may modify the terms at any time; the current version is available online and at the practice.
